日期：2015年7月13日至7月31日 研習方式：三週於 GT 正式修課、課餘安排活動

21 申請條件 大學部、或研究所在學學生。 有意提昇英語能力與國際觀者。

22 申請資料 研修申請表乙份 （至http://goo.gl/psQR3m 下載
或 索取， 並準備兩吋照片2張） 在學歷年成績單乙份 全系排名證明書乙份（研究生由系所認可）

23 評審標準 繳交申請資料 在校成績 英語檢定證明(可加分)

24 報名相關時程 報名截止日期：2015年4月10日(五)。 由清華大學工學院 陳愛婷小姐協助收件，中國 醫藥大學附設醫院 3D列印醫療研發中心公告並負責受理報名事宜。 公告錄取名單日期：暫定於2015年4月17日(五)公告 。 名額：正取5名，備取3名

25 費用 Program Fees 學程費用：美金 $3,200（台幣金額以匯款至美國當日匯率換算）
第一期費用新台幣＄31,000：2015年4月27日(一)前繳交。 第二期費用新台幣 $70,000：2015年5月4日(一)前繳交。 以上費用包括： 包括課程費、參訪費(可口可樂總部、CNN中心、馬丁路德金恩中心)、住宿費、美國當地接送機、預定行程文化參訪交通接送、六旗魔術山主題樂園(Six Flags Theme Park)門票、大聯盟勇士隊現場比賽門票…等費用。 以上費用不包括： 護照費、機票費、保險費、簽證費、行李超重費、餐費、醫療險及其他個人花費。

26 特別注意事項 機票：約NT$65,000，統一代為團購 護照：請儘速自行辦理
不可退費事項：於2014年5月12日之後因故不克成行者，預繳之學程費不可退費(因已匯至喬治亞理工學院) 生活費
