2 大綱 骨骼、肌肉系統疾病分類規則概要 疾分編碼與DRG之相關性

3 關節病變 代碼範圍：710-719 闗節炎(arthritis)：是造成關節發腫、疼痛、僵硬等毛病的總稱，而較常見的有
骨性關節炎(osteoarthritis；OA) 類風濕性關節炎 (rheumatoid arthritis) 痛風性關節炎(gouty arthritis)

4 關節病變 骨性關節炎(osteoarthritis；OA)(715.XX) 局部骨性關節炎 原發性(primary)：715.1x
續發性(secondary)：715.2x 局部性關節炎(localized arthritis )：715.3x 當醫師有明示部位但無提及為原發或續發性時以715.3X(包含雙側同一部位) 多發部位關節炎：715.8X

5 關節病變 例： osteoarthritis ，bilateral，knee Ans：715.36
osteoarthritis of knee and hip Ans：715.89

6 關節病變 外傷性關節炎(traumatic arthritis)：716.1x 是因關節受傷後所引起之關節炎 例：
traumatic arthritis ，left ankle ，due to old traumatic dislocation Ans： (late effect )+E929.X

7 關節病變 類風濕性關節炎(rheumatoid arthritis)：714.0 關鍵字：arthritis
類風濕性關節炎是一種免疫上的疾病，會造成 關節的慢性發炎，有時也會侵犯關節的周圍組 織或者是身體其他器官，症狀主要有關節紅、 腫、熱、痛。 關鍵字：arthritis

8 關節病變 痛風性關節炎(gouty arthritis)：274.0
痛風是一種身體內普林（Purine，或稱嘌呤）代謝異常的疾病，使血液裡尿酸濃度過高，醫學上稱為高尿酸血症，臨床上主要症狀為急性關節炎發作 。 痛風石(tophi)：274.8x 痛風如果沒有妥善接受治療的話，血液中過量的尿酸會於數年後於關節滑膜、軟骨、肌腱以及軟組織等處形成痛風石。

12 常見處置 全膝關節置換術(total knee replacement THR ) 代碼：81.54
因置換術無單、雙側之分，必須編兩個相同手術代碼，表示患者接受雙側置換術 患者因置換術之併發症(complication)，如感染(infection)、滑脫(loosening)等原因來接受治療。 區分患者為接受矯正置換術(revision)或只是移除置換物

13 常見處置 例： A patient was a victim of right hip OA，admitted and received right hip total hip replacement (THR) . When condition was stable then discharged . 診斷：715.35 處置：81.51

14 常見處置 例： The 52-year –old male patient was a victim of partial hip replacement s/p 2 years ago This time he was complained hip pain and x-ray showed prosthetic loosening. After admitted revision total hip replacement was performed 診斷： E878.1 處置：81.53 complication→mechanical→device→orthopedic,internal

15 常見處置 例： Removal of infected hip prosthesis with

19 背部病變 代碼範圍： Back pain Lumbago or low back pain ：724.2 Back pain not otherwise specified ：724.5 Psychogenic back pain：307.89 若確知造成back pain之原因，此back pain是屬於該病因之病徵時，則不再加編背痛之代碼。

20 背部病變 脊椎狹窄(spinal stenosis) ：723.0 或724.xx
黃韌帶肥厚，小面關節長骨刺及其囊肥厚，椎間盤突出使得dural space減少 due to 以下兩種狀況,則723.0 or 724.0x可 省略不編碼 (1) HIVD(722.xx)` (2) spondylosis(721.x) 若無due to ,則spinal stenosis 不可省略

21 背部病變 Ans：722.10 例： lumbar Spinal stenosis due to HIVD
脊椎狹窄若是因椎間盤脫位造成，則脊椎狹窄代碼可省略不編，只編椎間盤脫位代碼；但若二者無關聯性，則二者皆需編碼。 Q&A編號53

22 背部病變 椎間盤突出(herniation of interventral disc HIVD)代碼：722.XX
編碼時須注意是否有合併脊髓病變 HIVD with myelopathy:722.7x Traumatic HIVD視為dislocation by site 關鍵字：herniation

24 背部病變 spondylolisthesis 脊椎滑脫 一節椎體滑出另一節椎體 因退化性或外傷造成應編後天性代碼:738.4
未明示為先天性或後天性時應編:756.12

28 骨軟骨病變及後天肌肉骨髂畸形 骨髓炎(osteomyelitis)：730.0-730.9
為斜體字,不能為主診斷,需以原始病因碼為主斷 診斷若明示為何種細菌所致,需加菌種碼 當診斷無提及造成骨髓炎之明示原因，則 骨髓炎與糖尿病併存時,則視兩者有相關連 coding clinic2004 1Q p14

29 骨軟骨病變及後天肌肉骨髂畸形 例： osteomyelitis of left distal femur
DM,type 2 poor control Ans： diabetic with →osteomyelitis

30 骨軟骨病變及後天肌肉骨髂畸形 骨質疏鬆症(osteoporosis) :733.0x 第五位碼是以致病原因分類，如老年性或醫源性骨質疏鬆
Drug induced osteoporosis:733.09 use additional E code to indentify drug

31 骨軟骨病變及後天肌肉骨髂畸形 例： senile osteoporosis osteoporosis→ senile→733.01

32 骨折 代碼範圍: 病理性骨折:733.1x 創傷性骨折:800-829 顱骨骨折:800-804 頸和軀幹骨折:805-809
上肢骨折: 下肢骨折:

33 骨折 編碼原則: 一、骨折可分為close or open 1、close fracture :
(1)comminuted 粉碎性 (2)depressed 凹陷性 (3)elevated 舉起 (4)fissured裂縫 (5)fracture NOS未明示 (6)green stich 柳條狀 (7)impacted嵌入性 (8)linear線性 (9)march前進性 (10)simple單純性 (11)lipped epiphysis 骼滑脫 (12)spiral 螺旋形性

44 骨折 病理性骨折(pathologic fracture ):733.1x
當骨折被敍述為自發性(spontaneous)時,則以病理性骨折編碼 住院為治療病理性骨折,以病理性骨折為主診，引起病理性骨折之潛在性疾病為次診斷 fracture due to osteoporosis 應視為病理性骨折

45 骨折 例： pathological fracture due to metastatic carcinoma of bone ;ovary five years ago Ans： V10.4

46 骨折 例： open tibia shaft fracture ORIF+Debridement tibia bone
