2 相关要求： IDO暑期项目申请人面向UIC在读的大一、二、三年级学生，某些项目只针对某个年级的学生开放，请于报名前仔细阅读相关信息。 2. 所有项目成绩要求GPA2.0以上。某些项目 要求严格的英语成绩资格，请务必在放假期 间获取，报名时提交。

3 相关要求： 3.请随申请表附上身份证复印件一份和护照复印件三份，护照有效期需在项目结束之日起半年以后。 4. 已经报名的同学如因特殊情况需要退出，请提交有本人签名的书面申请，由提交申请之日计起已经发生的费用不予退还。

4 相关要求： 5.在项目进行过程中参加学生必须遵守UIC合作院校的校规和校外短期项目中的各项安排。如有个人行动计划应由父母一方出具书面申请，出发前两周交到IDO办公室，获得批准后才能成行，并承担由此带来的所有课业缺失和其他后果。 6.学生需知在国外修读的学分未必能全部或部分转回UIC，如因此对学习进度以及毕业时间有影响，或因报名此项目影响到自己其它的活动或补考，学生应自行合理安排并自己承担全部责任。

5 相关要求： 7.报名表家长签字栏必须是父母亲笔签名，接受传真复印件，如有假冒，报名表作废。 8.如果在身体和心理方面需要领队特别照顾，请一定在报名参加项目前跟IDO办公室如实提供目前以及历史状况，否则在项目期间发生任何有关问题自己承担全部责任，并负担校方为解决此问题产生的一切费用。情况严重者，校方有权终止继续参加项目并安排回国，由此产生的费用由学生自己承担。 9.报名培训/行政费用980元将在报名名单确认后一周内从农行帐户中收取。 。

9 Please understand that credits transferred from summer programmes will not be calculated into cGPA.

10 Passport and Visa Get your passport as early as possible (if you don’t have one). Make sure you have at least 6 months validity left on your passport counting from the August 1, 2015 (your passport should be valid at least until February 1, 2016). No individual visa application will be endorsed by UIC.

11 Training will be provided before visa interview.
Visa Application: Training will be provided before visa interview. UIC will arrange trips for students to go in group to attend the US/UK visa interview, etc. Make sure you have High School Graduation Notarized Certificate (高中毕业证公证书) ready for Canadian programme and ESADE Business School in Spain; kinship certificate (亲属关系证明）OR Birth Notarized Certificate （出生公证书） for the UK Middlesex programme before visa application.
